Physical Examination

Physicals are valid for 3 years from the date of the last exam.  Every athlete at SPA needs to have a current physical on file and uploaded through the Activity and Athletic Registration process, so please have your updated physical exam form ready to upload when prompted.  Link to MSHSL Physical Form

MSHSL Eligibility Forms

The MSHSL form needs to be filled out annually and is embedded electronically into the registration process.  Completion of the MSHSL form is required for all Upper School athletic teams.  As per MSHSL policy, athletes are not eligible to practice or play in games prior to the completion of this form.

Medical Information

During the registration process families are asked to provide medical and contact information.  Access to that information is restricted to the members of the athletic department, the athletic trainer, and the coaches of specific teams to use in the event of an emergency.  Coaches are only provided access to their team roster or for athletes in their program.
